I built an open-source online quiz and interview assessment project that you can download from GitHub, customize, and deploy on your own server.
It includes features for creating online assessments and supports proctored audio/video recording, making it useful for technical interviews, candidate assessments, coding evaluations, and internal testing.
This isn't a hosted SaaS platform - you get the source code and host it yourself.

- Resend - Sending assessment results by email
- PDF generation - Creating a candidate assessment report
